01 Research Background and Current Treatment Status

 

Treatment options for aortic arch pathologies include hemi-arch replacement with open distal anastomosis, total arch replacement, frozen elephant trunk prosthesis, and thoracic endovascular aortic repair (TEVAR). Complications of thoracic endovascular repair include stroke, spinal ischemia, endoleaks, and stent collapse, with stroke incidence rates ranging from 1.2% to 8.2%.

02 Left Subclavian Artery Reconstruction

 

Without the use of bypass, reconstruction of the left subclavian artery using chimney and window/branch stent techniques effectively reduces the incidence of post-TEVAR stroke from 5.6%-8.0% to 3.1%-5.3%.

03 Technique for ‘Type I Endoleak’ Induced by Balloon Dilation

 

Simultaneous balloon dilation and thoracic aortic stent deployment create a gap between the stent and the aortic wall, temporarily forming a Type I endoleak that helps maintain smooth blood flow through the branches of the aortic arch.

04 Intraoperative In Situ Fenestration for Reconstructing Aortic Arch Branches

 

Intraoperative in situ fenestration allows for precise reconstruction of aortic arch branches without the need for traditional open surgery, ensuring normal blood flow.

 

05 Postoperative Results and Technological Advantages

 

This technique is applicable to various types of aortic arch pathologies, boasting a high technical success rate and no early (≤30 days) complications, providing a safer and more effective treatment option.

06 Updated Treatment Guidelines

 

While the research shows promising early results, it remains retrospective with a small patient sample, lacking mid to long-term follow-up data and a control group.
